Patient-centered Care

Partnering with patients to plan and coordinate care is essential to providing quality care to each individual patient. As a nurse, I encourage expression of preferences, values, and needs by my patients. Below are two specific examples of projects focused on patient-centered care. 

My goal is to continually assess my patients to provide holistic care that meets at their needs. Spiritual care is often confused with religious care and may be avoided due to a lack of understanding. Patient-centered care includes providing spiritual care based on the patient's needs.

As a nurse, it is important for me to understand my community and to develop a plan of care centered around my patients. A patient may be an individual, an individual and their family, or an underserved community. 

Quality Improvement

Quality Improvement

Quality improvement engages nurses in constant assessment of care to continuously improve the safety and quality of patient care in any setting. Information is gathered and analyzed to assess the processes that can be changed to improve outcomes. 

A root cause analysis is a method of problem solving to find the source of problems requiring change to prevent future problems.

18 hours of learning was completed through the Institute of Healthcare Improvement covering topics on quality and safety.

Safety

Safety must constantly be assessed to minimize the risk for patient harm or harm to providers. Communication and effective use of safety strategies are key to promoting a safety culture.

Understanding current literature on HIPAA and patient safety is critical to providing the best possible care for patients.

I am passionate about improving patient safety and chose the topic for my scientific writing semester project. I was selected to present recommended practice changes to improve patient safety during medication administration at the 2018 ANCL Innovations Conference.

Teamwork and Collaboration

Teamwork and collaboration is essential to achieving quality patient care. Open communication, mutual respect, shared-decision making, and conflict resolution are all skills I have achieved through meeting this competency. These skills can be used within nursing or interdisciplinary teams to effectively function as a team to achieve patient care goals.

For patients with an Islamic background, it is important to understand how their religion affects their diets to ensure adequate nutrition. Nurses can collaborate with their healthcare team to establish a diet that meets the patient's  religious standards while fulfilling their health needs.

By working as a team, I was able to complete a collaborative research study with my peers on co-bedding NICU twins in the United States. We were able to work together towards a common goal of improving NICU outcomes in the United States.

Evidence-based Practice

EBP

It is imperative that I participate in structuring my work environment to integrate new evidence into the standards of practice. By using my clinical judgment and partnership with my patients, I can integrate evidence-based practice and patient values to deliver optimal healthcare.

By synthesizing current literature on physical activity for MDD patients, we were able to create a plan of care that incorporated the patient's desire to add exercise to his treatment plan.

Demonstrated ability to gather the best available evidence from reliable sources to present clinical guidelines for immunizations of children under 18 years of age.

Informatics

Information and technology are essential tools for safe and efficient patient care. Informatics requires continuous learning to stay current on tools that improve communication, prevent errors, manage information, and coordinate care. I have used the following EMR software as tools to provide the best care for my patients.
